Your family physician or nurse practitioner specializing in primary care (NPPC) provides holistic care and long-term follow-up, coordinating your care, monitoring your health, and taking preventative measures. They support you every step of the way, adjust treatments as needed, and facilitate access to resources and specialists when necessary.

For acute issues requiring rapid evaluation (examples: urinary or respiratory infections, sore throats, ear infections, sinusitis, abdominal pain, acute low back pain, sprains or minor injuries, mild allergic reactions, skin rashes).
The health assessment is a comprehensive consultation designed to evaluate your overall health, prevent the onset of disease, and optimize your long-term well-being. During this visit, we take the time to review your personal and family medical history, lifestyle habits (diet, physical activity, sleep, alcohol or tobacco use), and current concerns.
A complete physical examination is then performed to assess the different systems of your body. Depending on your age, risk factors, and current medical guidelines, we also recommend appropriate screening tests (blood work, cardiovascular screening, cancer screening, etc.).
Following this evaluation, we discuss the results with you, identify modifiable risk factors, and develop a personalized plan that includes prevention recommendations, lifestyle advice, and, if needed, medical follow-up or treatment.
This appointment is an essential step in maintaining good health and intervening proactively before more significant problems arise.

Secure video-conference virtual consultations are offered according to clinical indication.
Examples of forms, exams, and referrals: insurance forms, CNESST, SAAQ, disability or return-to-work forms; medical forms for employment or sports participation; medical notes, certificates, and school or professional forms; referrals to specialists (cardiology, dermatology, orthopedics, gynecology, etc.) and coordination of follow-up.

Private family medicine is a care model in which access to a family physician occurs outside the public system, allowing faster consultations, personalized follow-up, and greater flexibility in care organization. It emphasizes continuity, prevention, and comprehensive management tailored to your needs. The services offered are not covered by RAMQ and are therefore the responsibility of the patient or private insurance, as applicable.
Telemedicine can be appropriate for some follow-ups, medication renewals, medical notes, reviewing test results, and evaluating new symptoms when a physical exam is not required. If an in-person evaluation, vital signs, or onsite testing is needed, you will be offered an in-clinic appointment. Virtual care is not appropriate for urgent situations.
If your assessment suggests you need specialized care, your physician can initiate a referral and provide clinical context (history, exam findings, and relevant test results). You'll usually continue follow-up with your primary care team while waiting, and your plan may be updated once specialist recommendations are received. Referral timelines depend on urgency and local availability.
